Statewide COVID-19 Student Survey

Our middle schools and MLHS are participating in the COVID-19 Student Survey from March 8th–26th. This survey will be given to all students in grades 6 through 12 throughout Washington state.

The survey includes questions related to physical and mental health, thoughts and feeling pertaining to school during the pandemic; adult and peer support; attitudes towards and use of alcohol, marijuana, and other drugs and COVID-19 precautions. Survey results are used by teachers, school leaders, parents, community partners and policy leaders to help understand how to meet the needs of the youth in the state.

Participation in the survey is voluntary and the students’ answers are anonymous and each school will communicate additional information regarding the survey.

Please read the Parent and Student Notification FAQ document for more information about the survey:

This survey was created in an effort to collect student perception data to help school leaders in their end-of- year planning and student support programming, as well as information to aid in fall 2021 planning.

The COVID-19 Student Survey was created by the Office of Superintendent of Public Instruction (OSPI), the Washington State Department of Health (DOH), Health Care Authority (HCA), and the Psychiatry & Behavioral Sciences in the University of Washington School of Medicine (UW) to help capture the thoughts, feelings, and behaviors of students during the COVID-19 pandemic.

During this time, students have had to adjust to new ways of learning, socializing and living. The purpose of this survey is to help teachers, school leaders, parents, community partners and policy leaders understand how they can better help meet the needs of their youth.

This survey asks questions about mental and physical health, hope, social support from peers and adults, thoughts and feelings pertaining to school during the COVID-19 pandemic, attitudes towards and use of alcohol, marijuana and other drugs, and COVID-19 precautions.

The survey is anonymous, voluntary and will be administered to students in grades 6 to 12.

The survey will be administered completely online through the school and can be done in a school building or from home for students participating in remote learning.

Survey results will be shared with schools before the end of the 20-21 school year.
